TRICARE brochures and forms are located at Army Community Service, Community Readiness & Support Center, Bldg 119; Monday-Friday; 0700-1630.

Active-duty service members at Picatinny Arsenal are only covered by TRICARE Prime Remote; while Families have the additional option of selecting US Family Health Plan.  Either plan requires enrollment and the selection of a primary care manager, upon PCSing.  This can be accomplished at the Beneficiary Web Enrollment website, for TRICARE information on Beneficiary Web Enrollment click here.  US Family Health Plan enrollment forms can be picked up at ACS.

Active Duty Dental Plan - to schedule an appointment either call 1-866-984-ADDP or complete the Appointment Request Form.

TRICARE Dental Plan for family members, from this site you can enroll/disenroll, update your information and find a dentist.
