Parent Central

Parent Central and Outreach Services includes centralized enrollment for all activities as well as the waiting list for care.  To get on the Child Care Waiting list, please fill out the waiting list form and return it to building 3156.

Children using any of the Child and Youth Programs at Picatinny must be registered through our Parent Central Office.  There is no fee registration.  An official copy of the child's shot record and the following admission forms are required to enroll in Child and Youth Activities.  To shorten the in person registration process, you may print the forms and fill them out before coming to register at the Parent Central Office.

• CDS Health Assessment
• DD 2652 (Application for Department of Defense Fees)
  *Verification of Income LES is required.
• CYSS Health Screening Tool

 In addition to the above forms, you will be asked to fill out the following forms at the Parent Central:

• USDA Form
• CYSS Immunization Requirements
• New Jersey 4-H Club Member Registration Form
